Department of State Issues A New Form For J-1 Visa Waiver Application

In a surprise and quiet move at the beginning of this month (June 2002), the US Department of State (DOS) has issued a Form DS-3035 which replaces the State Department’s prior J-1 Visa Waiver Review Application. The new form became effective immediately.

The new form requires general information as well as J program information from the physician applying for the waiver and it is very similar to the previous form.

The application fee was raised from $ 136 to $ 230, and, as before, only a money order or a cashier’s check will be accepted.

The new form is available on the Department of State’s website at http://travel.state.gov/DS-3035.pdf.
